I'd say my success/failure rate is around 60/40. I've probably done 100 discs successfully with the software. But there's been a lot that are eligible for D2D and didn't work.
I just checked and I have the same version/UPC. Have you tried uninstalling and reinstalling the D2D beta software? I did that the other day so it's possible that could be a reason why it's working for me. See my post here:

      I had some failures today:
The Lost World: Jurassic Park [WS] [Collector's Edition]
This title is not eligible for Disc to Digital conversion

The Day the Earth Stood Still (1951)
There was a problem while verifying this disc.
Please try again later or contact Customer Care.

Pathfinder: Unrated [0-24543-45997-2]; unable to identify this disc.
I had a UV code with Black Hawk Down so I have that HDX but I stuck my disc in just to see and sure enough it gives Striking Distance. What an odd mix up. Some of the others make sense. Star Trek BDs all giving Star Trek 4 or Alexander giving 300 since they all exist in sets together. That one is baffling. I paid the 2 bucks just to see what it was and it's a Bruce Willis/Sarah Jessica Parker action flick. Weird.

Wow! Thanks for the tip. That was a big help. I just tried a couple of the "This disc is not eligible for Disc to Digital..." discs, and most of them worked.

          Entrapment
          The Tourist
          Public Enemies
          The Next Three Days
          The Brave One
          Body of Lies

Discs that had previously identified incorrectly (Identity, Taken, The Wrestler) still have that problem. But reinstalling the software seems to have helped the "not eligible" problem.

I'm going to try doing this tonight.

I tried Star Trek, Shooter, Body of lies, Robin Hood, Green Zone, etc... again hoping that they might work, but no luck.

Hopefully tomorrow I'll be able to post some success!

Yes, an emphatic YES! Un-installing & re-installing was the ticket. Was able to upload Blu Ray discs that were previously not eligible:

              Enemy At The Gates
              Airplane!
              Leatherheads

Still not recognized, but not reporting as ineligible, reporting as not recognized, please confirm the disc title, or take to Wal Mart:

              El Mariachi (Blu Ray)
              Fast Five (Theatrical) Blu Ray
              Rough Riders DVD
              Superman The Movie Blu Ray
              Superman IV Blu Ray

I am still searching and lurching for other discs that may now be eligible for Ultra Violet with the most recent update of eligible discs, but am very grateful to rdodolak for the tip to un-install and re-install D2D Beta. A huge THANK YOU to rdodolak.

I just tried Star Trek (2009), Robin Hood (2010), and Green Zone. Also did Body Of Lies last night. All four of those worked with the reinstalled software.

If you don't mind taking them to Walmart, it may work that way? I know I converted "Shooter" with no problems but I took them to Walmart for the conversion instead of the in-home D2D.

If you are talking about the new Star Trek movie. It worked for me. I have the 3 disc edition. Popped it in Friday i think it was. And no probs which is rare these days.

                        tbdave and Roxie_Muzic, you're welcome!

Yes, this seemed to have fixed a lot of issues, although not all, for discs which displayed the This disc is not eligible for Disc-to-Digital error message. I'm also re-reporting discs that can't be identified just in case those discs weren't being reported properly, if at all, due to whatever the underlying issue was.

Having problems with quite a few:

The Bourne Identity from Blu Ray (Says Ineligible)
The Bourne Supremacy from Blu Ray (Says Ineligible)
The Bourne Ultimatum from Blu Ray (Says Ineligible)
Star Trek: The Original Motion Picture from DVD - Comes up as Star Trek II, III, IV, and V. Says that the original isn't supported, but it's listed as supported on website.
Star Trek: The Original Motion Picture from Blu Ray - Says not supported
Star Trek III: The Search for Spock from DVD - Comes up as Star Trek II, III, IV, and V.
Star Trek III: The Wrath of Kahn from DVD - Comes up as Star Trek II, III, IV, and V.
Star Trek IV: The Voyage Home from DVD- Comes up as Star Trek II, III, IV, and V.
Star Trek V from DVD - Comes up as Star Trek II, III, IV, and V.
Star Trek Generations from Blu Ray - Comes up as Star Trek IV: The Voyage Home
Super 8 from Blu Ray - Says not supported

Seems to be a big trend with Universal and Paramount, did they just provide a lot of really broken data to the Disc-to-digital providers or what? These discs don't read correctly with any of them. It's really a nuisance, and I don't want to have to drive way out of my way to hit a physical store and deal with the service in-person.

Sounds like there was some bug with the installation of an upgrade that's causing issues. Re-install seems to fix it. Not something I want to tackle while I'm traveling.

Anyone do the re-install with downloaded movies? Any issue with playing them afterword?

Every movie I had a problem with worked after a re-install. Who knew it would be so easy to fix. Now to get my Wild Things BD back from the friend I let borrow.

Did reinstall...tried Pet Semetary, Out of Sight, Repo Man criterion, Hudsucker Proxy, Moon. Out of Sight and Moon finally worked but the others did not. Also tried Mulholland Drive (german region free disc) it doesn't work but the Uk Miami Vice (theatrical cut) did work this time.
